Acute Care Services

Euroa Hospital provides care and is able to admit patients with and without private health insurance and is registered to accommodate 21 beds and 3 Urgent Care beds.

We provide care to palliative care patients, cardiac patients, those requiring diabetes management, rehabilitation and those awaiting placement in our very own Residential Aged Care facility GraniteHill or facilities in the surrounding districts.

The Euroa Hospital has achieved the following Private Hospital Registration (1st July 2023 to 30th June 2025) and National Safety and Quality Health Service (NSQHS) Standards Accreditation Certificate (27th June 2023 to 26th June 2026).   Euroa Hospital can be contacted on 03 5795 0221 and entry is via Weir Street Euroa.

Euroa Health works closely with Goulburn Valley Health to provide both Transitional Care Program and Restorative Care Program for patients of all ages in our community which enables patients to receive medical care close to home and their support networks.

Transitional Care

What is Transitional Care?

The Transitional Care Program provides nursing support, personal care, physiotherapy and other allied health disciplines, medical support and case management. TCP can be provided as a bed based service, either in low or high level residential aged care or as a home based service.

All TCP care recipients work with their case manager and care team to establish their goals and develop their plan of care. It is expected that this plan will be regularly reviewed and updated as their care needs change.

A TCP is time-limited. The amount of time required will depend on an individual’s situation. However, the most common scenarios are as follows:

Usually people stay on the program for six to eight weeks and the limit is 12 weeks. Within this time, it is expected that people will have been assisted to access suitable longer-term care and support.

If further therapeutic benefit is possible, the program may request an extension from the Aged Care Assessment Service (ACAS) for a maximum period of 42 days (or 6 weeks). A further extension cannot be requested for a person who has already received the maximum extension during a particular period of care.

Restorative Care

What is the Restorative Care Program?

Restorative Care Program aims to improve your independence and confidence to return to everyday activities after surgery, a short-term illness, a relapse of a chronic condition, or while planning a safe discharge. While in the program, you can access a range of health and lifestyle services designed to meet your needs.

Services are offered as part of an ongoing care plan, giving you and your family time to consider and plan for your long-term care needs; this may include returning home with community support or becoming a resident in an aged care facility.

You will be supported by a Care Coordinator during the Program and the role of your Care Coordinator is to work with you and your family to identify what services you require, to meet your goals and assist you and your family with your discharge plans.

Services may include personal and/or nursing care (showering and dressing assistance), medical care, meals, transport to/from appointments, lifestyle activity program, and low-level therapies such as physiotherapy, occupational therapy, speech therapy, podiatry and dietetics.

Is it for me?

Eligibility for this program is based on your support needs and age. To be eligible for this program, you must be aged:

  • 65 years or older (50 years or older for Aboriginal or Torres Strait Islander people), or
  • 50 years or older (45 years or older for Aboriginal and Torres Strait Islander people) and on a low income, homeless, or at risk of being homeless.

 

Who provides the services?

To make sure you can access the help you need at an affordable price, the government subsidises a variety of organisations across Australia. These organisations are called service providers. They deliver care and services to you at a subsidised price. Euroa Health has been accredited as a provider to deliver CHSP services to the community.

How much will it cost me?

If you are eligible, you’re expected to contribute to the cost of your care if you can afford to. The cost depends on the type of service you require the current client contribution at Euroa Health (put link here for cost table)

How long will it take to get some help at home?

First you will need to be assessed. After that a member of the team will review your referral and contact you to discuss expected waiting times.
